Notes: The inaugural year was 1509. Runners often ran using aliases; their actual names are provided in parentheses when known. The race was not officially held for 1915-18 due to World War I, not held in 1940-46 due to World War II, and in 1952 due to an outbreak of "foot and mouth" disease. The 1916 unofficial race included a female participant. From 1966, only residents in the Parishes of Carwath, Carstairs, Libberton, Walston, Dunsyre, Dolphinton and Elsrickle have been allowed compete. In 1980, a rule to the effect that anyone winning the Red Hose in three successive years shall be ineligible to compete in the race for two years. Times were not recorded for some years, indicated by "n/r".

In 2011, the first finisher was Ivan Scott (IRL) but, as he was not a local runner, he was not declared the winner.
In 2014 and 2015, the first finisher was Luke Foss but, as he was not a local runner, he was not declared the winner.

In 1508, King James IV gave a Charter of the Lands of Carnwath to John, third Lord Somerville, on the following terms:

"Paying thence yearly....one pair of hose containing half an all of English cloth at the feast of St John the Baptist, called Midsummer, upon the ground of the said barony, to the man running most quickly from the east end of the town of Carnwath to the Cross called Cawlo Cross....."

The race must still be run, however, to meet the requirements of the Crown Authorities whose written permission is necessary before the performance of the service can be suspended; as for example, during a national emergency such as war, and in the case of a local exigency such as the outbreak of Foot-and-Mouth Disease. In fact these are the only instances known in which the Crown Receiver has sanctioned its suspension.
